Hormone Replacement Therapy: What Every Woman Needs to Know About Perimenopause and Menopause
January 26, 2026
Every year around New Year’s, I take stock — not just of my own health and priorities, but of the questions women are asking most loudly.
This solo episode of Midlife Realignment exists because the same themes kept coming up in my DMs, my comment sections, and my practice:
- “Why is my doctor afraid of hormones?”
- “Am I being irresponsible if I don’t take HRT?”
- “Is it too late for me?”
- “Why do I feel like I know more than my provider?”
If you’ve ever walked out of an appointment feeling dismissed, confused, or intimidated by a white coat — this episode, and this post, is for you.
Education Is Not Fear-Mongering — It’s Empowerment
One of the most frustrating narratives I deal with online is the accusation that talking openly about hormones is “fear-mongering.”
Let me be very clear.
I never tell women:
- “You must do this.”
- “If you don’t take hormones, something terrible will happen.”
What I do tell women is:
- Here’s what the science says
- Here’s what decades of clinical observation show
- Here’s what the research continues to confirm
There is a massive difference between coercion and informed choice.
Ironically, many of the loudest critics of hormone education don’t use hormones themselves — which tells me that what they’re sharing is personal bias, not data.
Why Most Doctors Are Not Hormone-Literate (And It’s Not Their Fault)
This is one of the most important realities women need to understand:
Do not expect your regular provider to be hormone-literate.
Medical students currently receive, at most, about one week of menopause education — and much of that training still centers on:
- Birth control pills
- Antidepressants
- Outdated fear around estrogen
Meanwhile, the FDA has approved estrogen, progesterone, and vaginal estrogen — and even removed black-box warnings from all estrogen products at the end of 2025.
So we have a bizarre disconnect:
- Hormones are FDA-approved
- Providers are not trained to use them
This is not a failure of your doctor.
It is a failure of the system.
How to Find a Truly Hormone-Literate Provider
If your provider responds to hormone questions with blank stares, fear-based statements, or birth control prescriptions — it is appropriate to seek another opinion.
Here’s how I guide women:
- Call a compounding pharmacy in your state
Ask which providers they trust who prescribe bioidentical hormones. - Ask the right questions:
- Do you prescribe transdermal estrogen or testosterone?
- Do you use oral micronized progesterone?
- Do you use hormones as first-line therapy — not pellets or injections?
- Know what’s not first-line:
- Birth control pills
- Pellets
- Long-acting injections
Hormone literacy is about physiology, dosing, and delivery method — not credentials alone.
Hormones Are Not the First Step — But They Are a Powerful One
Despite what critics assume, hormones are not the first thing I reach for in practice.
Foundations always come first:
- Gut health
- Blood sugar regulation
- Liver function
- Stress management
I’ve seen hot flashes resolve simply by restoring gut integrity and lowering cortisol.
That said, when hormones are indicated — they are game-changing.
They are not cosmetic.
They are not anti-aging fluff.
They are chemical messengers that influence:
- Sleep
- Cognition
- Bone density
- Cardiovascular health
- Insulin sensitivity
- Pelvic floor integrity
Ignoring them when they’re needed is not “natural.”
It’s neglectful.
Muscle, Strength Training, and Longevity in Menopause
Muscle is not optional in midlife.
It is an organ of longevity.
Strength training:
- Improves insulin sensitivity
- Protects bone density
- Reduces fall risk
- Increases mitochondrial density
- Supports cognitive health
You do not need heavy barbells to start.
Bodyweight, bands, Pilates, yoga — all count.
Consistency beats intensity.
Always.
Sleep, Progesterone, and Early Intervention
One of the earliest clues that hormones are shifting is sleep disruption, especially in the luteal phase.
For some women:
- Supplements help
- Lifestyle helps
For others, progesterone restores sleep almost immediately.
Waiting years to intervene out of fear doesn’t earn you a prize.
It just prolongs suffering.
Vaginal Estrogen: The Most Underused Menopause Tool
Genitourinary symptoms of menopause are nearly universal:
- Vaginal dryness
- Burning
- Pain with sex
- Urinary urgency
- Recurrent UTIs
Vaginal estrogen:
- Is ultra-low dose
- Stays localized
- Does not raise systemic estrogen levels
- Is safe even during cancer treatment
It preserves tissue integrity, collagen, nerve endings — and quality of life.
Suffering in silence is optional.
Is It Ever Too Late to Start Hormones?
No — but timing matters.
The greatest protective benefits occur when HRT is started:
- Within 10 years of menopause
- Or during late perimenopause
Early intervention:
- Reduces fracture risk
- Protects cardiovascular health
- Preserves cognition
Estrogen is the single most powerful protector against bone fractures in women — a fact beautifully documented in Estrogen Matters.
Bone density loss is silent.
Cognitive decline is slow.
Prevention must start early.
What If You Choose Not to Take Hormones?
That choice deserves respect.
If you opt out of HRT, you must be extra vigilant about:
- Annual labs (lipids, glucose, insulin, CRP, thyroid)
- Bone density scans starting around age 45
- Strength training
- Protein intake
- Gut health and absorption
Hormones are not the only path — but they are a protective one.
Clotting Disorders, Cancer Risk, and Individualized Care
Transdermal hormones carry no increased clotting risk, even for women with genetic variants like Factor V Leiden.
Cancer risk must be individualized — not generalized.
I work closely with hormone-literate GYNs like Dr. Suzanne Fenske, who calculate actual risk rather than relying on outdated fear.
Most women, even with family history, are eligible for safe hormone support when evaluated properly.
The Bottom Line
Menopause is natural.
Suffering is not.
Hormone education is not fear-mongering.
It is liberation.
When women understand:
- Their biology
- Their options
- Their timing
- Their right to choose
They stop outsourcing authority — and start thriving.
That is why Midlife Realignment exists.
